We are pleased to announce that online registration forms are now available for the current year's York High School and York Middle School Athletics. All students wishing to participate in any athletic activity this year must complete the required forms as follows:

York High School student-athletes ("Proceed to Online Forms" button below; choose the following two documents)
a) Athletics Participant Form - HIGH SCHOOL ONLY
b) Emergency Information - HIGH SCHOOL ONLY

York Middle School student-athletes ("Proceed to Online Forms" button below; choose the following document)
a) Athletics Participant Form - MIDDLE SCHOOL ONLY
b) Emergency Information - MIDDLE SCHOOL ONLY

Upon finishing these forms, ensure that your child has an active sports physical examination on file with the Athletic Department. Physicals are active for 13 months from the date of the exam. Updated paperwork can be submitted as follows:

*For York High School students please email updated paperwork to [email protected],fax to 207-351-4001 (this is a YHS Athletics specific fax line; please encourage the provider’s office to use), or drop off a hardcopy at the YHS front office.

*For York Middle School students please send to [email protected], faxed to 207-363-1815, or dropped off at YMS front office.
